Our multidisciplinary team of healthcare professionals offers a comprehensive range of services to all eligible patients related to terminal illness:

Medical care

Skilled healthcare professionals, such as physicians and registered nurses, provide superior medical care to address the needs of the patient.

Personal care

Certified Nurse aide assists with personal hygiene, bathing, grooming, and other daily activities to maintain the patient's comfort and dignity.

Volunteer Service

Volunteer(s) to help with errands and/or provide companionship.

Therapy Services

We provide Physical therapy, Occupational therapy, and Speech therapy services in the patient's home, as well as dietary counseling and support for the patient and family when medically indicated.

Continuous Home Care

For crisis management at home with intensive home nursing care.

Emotional and Psychosocial Support

Trained counselors, social workers, and chaplains offer compassionate emotional support and counseling to patients and their families, addressing their psychological, spiritual, and social needs during this challenging time.

Palliative Care Program

We focus on providing comfort and relieving pain through optimal Palliative care techniques, prioritizing the highest possible quality of life for the patient.

Respite Care

Short-term care services are available to give temporary relief to family caregivers, allowing them to rest or attend to personal needs while their loved one receives professional care.

Bereavement Support

We provide bereavement support to families and loved ones after the patient’s passing, offering counseling and resources to help them cope with grief and loss.

Short-term Inpatient Care

For management of acute symptom exacerbation.

Care Coordination

Our hospice team collaborates closely with the patient’s primary care physician and other healthcare providers to ensure consistent and coordinated care, meeting the unique needs of each individual.

Medical Equipment and Supplies

We help arrange and provide necessary medical equipment and supplies, such as hospital beds, wheelchairs, and oxygen supplies, to ensure the comfort and safety of our patients at home.

Cardiac Program

North Gwinnett Hospice Services, LLC has created the Touch of the Heart Cardiac Program to assist our patients living with advanced cardiac diseases such as advanced heart failure. This program is to improve our patient's quality of life, reduce the need for hospitalization, and respect the dignity of our patients.

Pulmonary Program

North Gwinnett Hospice Services, LLC has created a Breathe Easier Pulmonary Program to focus on our patients living with advanced pulmonary diseases such as advanced COPD. We understand that for patients living with respiratory disease, taking simple breaths can be very distressing and painful.

Namaste Program

Namaste Care is a holistic approach to caring for patients with advanced disease by emphasizing, connecting, and honoring the spirits within each individual.
